American Traveler is hiring an RN Case Manager for a Hospice position requiring one year of hospice experience and an active CA RN license.

Home health and hospice setting covering San Francisco, San Mateo, and Santa Clara counties

Schedule runs Sunday through Thursday with flexible hours between 10am-7pm or 11am-8pm based on patient availability

Active CA RN license required

Current BLS certification required

Minimum of 1 year of Hospice experience required

Admit patients to hospice services and assess physical, psychosocial, environmental, safety, and developmental needs

Develop individualized plans of care based on comprehensive patient assessments

Document observations and interventions related to patient environment and care

Coordinate care with a multidisciplinary team

Participate in the hospice program's quality assessment and performance improvement initiatives

Conduct follow-up visits as needed
