Mobile Outpatient
In this position, you’ll see patients in a mobile outpatient setting. Our patients are not homebound but prefer the convenience and comfort of receiving therapy at home. This model allows for meaningful connections, individualized treatment, and functional interventions in real-life environments. Here you will manage your own caseload and schedule with support from your team and Director.
What You’ll Do
- Evaluate and/or treat participants and create individualized, goal-driven care plans
- Deliver skilled therapy focused on mobility, strength, balance, and fall prevention
- Provide care in a group-based, collaborative environment
- Complete timely, compliant documentation
What We’re Looking For
- Active NC Physical Therapist and/or Assistant license
- Experience with adult/geriatric populations preferred
- Strong clinical decision-making and independence

Our ideal candidate has at least 1 year experience in adults/geriatrics, is reliable, has good communication skills, is empathetic and patient and can build a meaningful relationship with their client. Someone who can provide high quality personalized treatments in a one-on-one setting.
Responsibilities
- Identify and meet patients’ goals and needs
- Develop care plans using a variety of treatment techniques
- Provide quality, personalized and evidence-based care and proven interventions
- Motivate patients during treatment in order to help them function optimally
- Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
- Consult and practice with other health professionals
- Evaluate effects and monitor and communicate progress
- Document patient care services
- Management of PTAs
Skills
- Proven work experience as a Physical Therapist or PTA
- High level of autonomy and independence
- Ability to manage patients with different types of personalities
- Current knowledge of treatment practices
- Drive to continually learn and grow
- Professional behavior
- Excellent interpersonal communication skills
- Valid license to practice physical therapy in NC
- Good documentation abilities and completion time
