Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Cerner and Athenahealth.
- Administer medications via injection, IV infusion, and other routes following aseptic techniques and HIPAA regulations to ensure patient safety.
- Perform specialized procedures including catheterization, tube feeding management, ostomy care, and wound dressing changes with precision and attention to infection control standards.
- Collaborate with multidisciplinary teams on discharge planning, case management, and PT/OT/ST, and social workers to coordinate effective patient care across inpatient, outpatient, home care, and long-term care settings.
- Manage emergency situations by providing prompt care and communication, acute pain management, CPR, Falls.
- Educate patients and families on health coaching topics such as geriatrics, dementia care, tube feedings, intravenous medications, and chronic disease management
- Ensure accurate medical documentation of care performed, medication administration records, lab results, intakes and outputs, admission intakes, weekly skin monitoring, daily skilled assessments.
Requirements
- Valid RN license with current registration in the relevant jurisdiction.
- Valid CPR card
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for effective patient assessment and problem management
- Ability to work with and delegate responsibilities among nursing assistants and Medication Technicians
