Job Summary
We are seeking a dedicated and energetic ACTT (Assertive Community Treatment Team) Qualified Professional to join our dynamic mental health and social services team. Duties
- Conduct thorough patient assessments, including mental health evaluations, substance use screening, and physiological observations using medical terminology and diagnostic tools.
- Develop and implement individualized treatment plans that incorporate behavioral health strategies, discharge planning, and community resource coordination.
- Provide crisis intervention and behavioral health counseling tailored to clients with mental health conditions, substance use disorders, or disabilities.
- Manage electronic health records (EHR) accurately while ensuring compliance with HIPAA regulations to protect patient confidentiality.
- Collaborate closely with multidisciplinary teams—including hospital staff in Level I Trauma Centers or ICU settings—to facilitate inpatient, outpatient, post-acute care, and managed care services.
- Prioritize tasks effectively in high-pressure situations such as emergency medicine or acute care environments while maintaining excellent patient service standards.
- Coordinate with families and caregivers to support client communication, discharge planning, and community resource linkage for holistic care delivery.
- Engage in diagnostic evaluations and clinical case management to monitor patient progress and adjust treatment strategies accordingly.
Requirements
- Proven hospital experience with a focus on acute care settings such as ICU or emergency medicine environments.
- Experience working within Level II or Level I trauma centers is highly preferred.
- Demonstrated expertise in behavioral health, mental health counseling, addiction counseling, or social work—particularly with children or individuals with disabilities.
- Strong knowledge of physiology, medical terminology, and patient assessment techniques essential for effective case management.
- Proficiency in EMR/EHR systems along with understanding of HIPAA regulations to ensure secure handling of medical records.
- Valid Basic Life Support (BLS) certification; CPR certification is required for immediate response scenarios.
- Ability to work with diverse cultural backgrounds and individuals from various socioeconomic statuses with sensitivity and professionalism.
- Excellent task prioritization skills combined with outstanding client communication abilities—especially when working with families or during intake processes.
- Experience managing patients with substance use disorders or behavioral health conditions in outpatient or inpatient settings is highly valued.
