Job Overview
We are seeking a dedicated and compassionate Home Care Nurse (RN/LPN) to provide high-quality nursing services in patients' homes. Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to develop individualized care plans.
- Administer medications, injections, IV infusions, and other treatments in accordance with physician orders and established protocols.
- Monitor patient progress and adjust care strategies as needed to optimize health outcomes.
- Provide specialized care for individuals with complex needs such as Alzheimer's disease, dementia, or developmental disabilities, ensuring safety and comfort.
- Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques.
- Maintain accurate medical documentation within electronic health record (EHR) systems while adhering to HIPAA regulations.
- Collaborate with interdisciplinary teams including physicians, therapists, and social workers to coordinate comprehensive care delivery.
- Ensure infection control practices are followed rigorously to prevent healthcare-associated infections.
Qualifications
- Valid RN or LPN license with current state certification; additional certifications such as Basic Life Support (BLS) are required.
- Demonstrated experience in home health care or related settings such as hospitals, nursing homes, assisted living facilities, or hospice is highly desirable.
- Proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient management.
- Strong clinical skills including patient assessment, medication administration (including injections), catheter care, and tube feeding management.
- Knowledge of ICD coding (ICD-9/10), CPT coding, and medical terminology to ensure accurate documentation and billing processes.
- Experience working with diverse populations including pediatrics (neonatal ventilator care), geriatrics (memory care), and individuals with disabilities or complex chronic conditions.
- Excellent communication skills coupled with critical thinking abilities to triage effectively and respond promptly to urgent situations.
