Job Title: Supportive Services Manager
Program/Dept. Assisted Living Program

Reports to: Senior Supportive Services Manager
II. Job Summary:
The purpose of this position is to provide leadership and oversight of Clare Housing’s supportive housing program. This position coordinates on-site service delivery for adults living with HIV/AIDS and other disabilities. The major challenges of this position are to deliver services to clients with complex health, mental health and substance abuse issues all in one building. Supervising a 24-hour staff of 8 to10 full-time and part-time employees of the supportive services program is also challenging.
III. Core Job Functions:
1. Conduct outreach and manage application and intake process for supportive housing program (both
assisted living clients and tenants).
- Coordinate resident selection, move-in, and orientation process.
- Work collaboratively with the property manager on admissions
- Keep manageable balance of assisted living versus tenants in building.
- With director of nursing, assist with assessments for admission to 24-Hour Customized Living
program.
2. Develop and coordinate groups and other programmed activities at facility.
* Coordinate support groups, therapy groups, social activities.
* Procure additional professional services (chemical health assessments, on-site mental health
services, etc.) as needed per site.
3. Coordinate and implement housing service plans and/or CADI Service Agreements with residents.
- For assisted living clients, negotiate services and rates between Clare Housing and with the
county or contracted CADI workers. Maintain ongoing communicates with CADI workers as client needs
change.
- Provide oversight, along with nursing, that excellent quality of services are being delivered to
client.
- Provide information, support, referrals and advocacy to all residents in building.
- Make referral to and collaborate with Housing Stability Specialist for appropriate
clients.
- Develop Eviction Prevention Plans for residents who are at risk of losing their housing. Provide direct advocacy with landlord.
- Communicate regularly with HIV case manages and other service providers to coordinate services to tenants.
- Respond to behavioral emergencies and intervene in crises as necessary.
4. Submit quarterly and/or annual government contract work Conduct client interviews and coordinate data collection process (HMIS) in accordance with project compliance and evaluation activities.
5. Supervise resident assistants.
- Recruit, interview and hire (with RN) Resident Assistants.
- Orient, schedule, supervise and evaluate all resident assistant staff in conjunction with Lead RN and/or Director of Nursing.
- Address performance issues as needed in a professional manner.
- Facilitate regular staff meetings.
- Provide on-call coverage, during and outside of normal working hours, if necessary.
6. Serve on agency committees and task forces as requested.
Relationships
This position reports to the program director. This position supervises the resident assistants.
IV. Position Requirements:
Minimum Education and Experience
A bachelor’s degree in the behavioral sciences or master’s degree is preferred. A new incumbent in this position should possess a minimum of five years’ experience in nonprofit human services work, with at least three of those in a supervisory or managerial position.

- Ability to identify the nature of problems and to create plans to address the problems.
- Knowledge of and ability to conduct effective outreach activities.
- Ability to write cogent and concise case notes, reports and treatment plans.
- Ability to maintain confidentiality in both client and employee information and privacy of residents.
- Ability to work with a multi-cultural diverse population with multiple health and mental health diagnoses.
- Ability to maintain professional boundaries and practice self-care in a position that is confronted often with physical and mental illness and chemical abuse issues.
- Proficient in Microsoft software applications and HMIS data collection. Certifications/ Requirements:
- CPR/First Aid Certification
- Medications Administration Training
- MN Divers License or MN State ID
- Pass MN Department of Human Services Back Ground Check.
V. Working Conditions and Environment:
This position is located in a supportive housing building for adults living with HIV/AIDS and other disabilities. Many of the residents are formerly homeless individuals. The building is accessible and smoke free. This position works during normal business hours which may include evening and weekend hours.

Pay: $60,000.00 - $65,000.00 per year
