Full Time or Part Time Available!
Family Home Health is seeking a Physical Therapist who is ready to make a difference in the lives of older adults. You will be traveling to East Brady, Rimersburg, New Bethlehem, and surrounding areas. We believe that every staff member who comes in our doors is a part of our family.
Essential Functions include but are not limited to:

Participate in the development of the total plan of care.

Maintain appropriate clinical records, clinical notes, and report to the physician any changes in the patient’s condition. Submit these records in accordance with the organization policy

Maintain contact / communication with other personnel involved in the patient’s care to promote coordinated, efficient care
