On Call Licensed Vocational Nurse (LVN) – Memory Care
Location: Pleasant Hill, CA
Pay Range: $36.00 - $38.00 per hour
Memory Care of Contra Costa is seeking a Licensed Vocational Nurse (LVN) to join our team. This person will ensure provision of quality resident care, promote resident wellness, and provide direct care services to the residents ensuring optimal health and safety.

Maintain knowledge of current assisted living regulations. Ensure compliance with community policies/procedures, pharmacy policies/procedures, and state and federal agency regulations.

Protect resident and staff safety by recognizing, reporting and taking immediate action to minimize risk of accident or injury. Report incidents according to community policy and state and federal laws.

Observe for any signs of resident abuse or neglect and take immediate action to prevent harm.

Assure all infection control guidelines are met and appropriate tracking and surveillance procedures are followed according to community and state requirements.

Assure medication associate training and monitoring meets regulatory requirements and community quality standards.
Job Requirements

Current CA LVN License
-
Minimum of one year experience in assisted living or health care.

Current CPR certification
-
Any other state specific certifications as required by the regulatory bodies governing the community.
