CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care is seeking a seasoned Registered Nurse (RN) for our Home Health Department to provide skilled nursing care for clients of all ages in their private residences.
Job duties include, but are not limited to:
- Provides skilled nursing care to clients in their personal residences throughout the DFW area.
- Coordinates care with interdisciplinary team to ensure optimal care for and benefit to each client.
- Communicates with clients, family members, interdisciplinary team members and management regarding client status.
- Prepares and administers medications and treatment as ordered by physician.
- Assists with and participates in development and implementation of written care plans.
- Ensures the accurate and timely documentation of all client treatment, medication and other pertinent information.
- Performs other duties as assigned.
Minimum qualifications include:
- Associate or BS in Nursing.
- Possession of a valid license as a Registered Nurse (RN) in good standing with the State of Texas.
- 2+ years of recent experience as an RN in a clinical setting.
- Experience using Axxess Care.
- Proficiency in Kinnser Software is highly preferred.
- Proven knowledge of general, rehabilitative and restorative nursing and medical practices, procedures laws and regulations/guidelines governing Home Healthcare.
- Excellent verbal, written and interpersonal skills.
- Proven computer skills for performing extensive patient documentation/charting.
- Valid Driver’s License with reliable form of transportation to drive throughout the DFW area to client residences. Auto Liability Insurance also required.
- 1+ years of In-Home Healthcare preferred but not required.
