This position provides targeted case management services for individuals supported through the Title 480 Home and Community-Based Medicaid Waiver Program and the Title 471 Senior Care Options Program. You’ll conduct assessments, determine eligibility, develop personalized care plans, coordinate services, and monitor progress to ensure each individual receives the support they need to thrive. Four (4) openings - these positions will be at the Lincoln office.
Desire: Bachelor's or Master's degree in Human Services, Education or Health/Medical field OR Registered Nurse and minimum 2 years professional experience in one of the following fields: long term care, gerontology, rehabilitation, health/ disability case management, children with special health needs or health/medical.
Minimum Qualifications/Necessary Special Requirements:
Graduation from an accredited four-year college or university with major coursework in social work, gerontology, psychology, human services, or a specified field of study to fulfill state-mandated qualifications plus two years of experience in providing program services or case management to a geriatric or disabled population including six months of supervisory experience; or any equivalent combination of training and experience that provides the desirable knowledge, abilities, and skills that meet the state mandated requirements.
NECESSARY SPECIAL REQUIREMENTS:
Possession of a valid driver’s license when operating a vehicle is necessary to the satisfactory performance of assigned duties.
