Job Overview
We are seeking a dedicated and experienced skilled nurse to lead our care team in providing exceptional care. The ideal candidate will possess a strong background in nursing administration and acute care, with a passion for improving patient outcomes and ensuring patient rights are upheld. This leadership role requires a commitment to fostering a supportive environment for both staff and home bound patients while overseeing the development and implementation of care plans.
Duties
- Oversee the care team, ensuring compliance with all regulatory standards and best practices.
- Supervise care staff, providing guidance, training, and performance evaluations.
- Develop, implement, and evaluate individualized care plans for patients.
- Ensure that patient rights are respected and upheld at all times.
- Collaborate with interdisciplinary teams to promote optimal patient care and outcomes.
- Maintain up-to-date knowledge of nursing practices, physiology, hospice care, and home care services.
- Lead initiatives aimed at improving quality of care within the facility.
- Recruit for care team to ensure that business has enough care staff to be able to ensure adequate coverage and support for our patients.
Experience
- Proven experience in nursing leadership or administration within a Home Health Agency
- Experience in a Medicare, Medicaid and Insurance setting.
- Strong knowledge of patient's rights, nursing regulations, and best practices in patient care.
- Experience in supervising care staff and managing teams effectively.
- Familiarity with developing care plans tailored to individual patient needs.
- Background in hospice care or home care services is a must.
- Excellent communication skills with the ability to foster relationships with staff, patients, and families.
