Case Aide
Position Summary: BEDS Plus Care provides services to the homeless and those at imminent risk of homelessness in southwest suburban Cook County. The Medical Respite Case Aide will support families in the transitional housing program including victims of domestic violence.
Primary Responsibilities:
Agency Intake:
❖ Take calls from clients in need of agency programs, ask screening/qualification questions.
❖ Refer to appropriate agencies or begin application process by sending application packet.
❖ Maintain knowledge of all programs and related community programs and services.
❖ Provides support and direction for people as they transition from homelessness to housing
v Managing intake hours at partner agencies (Cook County Health System, local municipalities) and BEDS’ facilities.
❖ Complete Coordinated Entry referrals on an as needed basis
❖ Directly work with a team of advocates and support staff
❖ Advocate for program participants as they search for supportive and/or independent housing and provide oversight of internal housing programs.
❖ Help participants create support systems and participate in the community as they desire.
❖ Provide independent living skills instruction to shelter residents
❖ Maintain relationships with services providers, volunteers, staff and community members.
❖ Maintain regular contact with the supervisor and shelter support staff to apprise them of resident progress and to assist in the resolution of potential problems.
❖ Work with clients on a one-on-one basis to provide education, guidance and referrals tailored to each client’s unique circumstances and create housing stability plans
- HMIS Entry, documentation of Street Outreach CLS and Coordinated Entry Progress trackers.
- Housing Navigation
- Acceptance and facilitation of CoC call center referrals and community -based call/walk-ins.
- Offer follow-up on hospital referrals for medical respite.
- Meet immediate and basic needs including but not limited to food, clothing, transportation.
- Attend relevant community-based meetings and work groups.
Required Skills:
· Skills or experience in the areas of homeless issues, mental health, or substance abuse.
· Ability to work with people of diverse backgrounds, experiences and challenges.
· Availability, dependability and willingness to be flexible.
· Demonstrated ability to exercise patience, solve problems and think strategically.
· Ability to use Microsoft office including Excel and Word programs.

Education and Qualifications: Bachelor's degree in related population,
Hours and Location: Schedule must be approved by supervisor. Evening and weekends expected based on client needs. Travel required.
Job Type: Full-time
Pay: $20.00 - $24.00 per hour
