Job Overview
We are seeking a dedicated and energetic Home Health Physical Therapist to join our dynamic healthcare team. Responsibilities
- Assess patients’ physical conditions and develop individualized treatment plans that align with their health goals and medical needs.
- Deliver expert physiotherapy interventions, including therapeutic exercises, manual therapy, trigger point therapy, and mobility training.
- Conduct discharge planning to ensure seamless transition from active therapy to ongoing self-care or community resources.
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient outcomes.
- Maintain accurate clinical documentation standards within electronic health records (EHR) systems, including progress notes and treatment updates.
- Manage home & community care activities by educating patients and caregivers on exercises, safety precautions, and post-treatment care.
- Participate in outpatient clinic or inpatient hospital experiences as needed to support continuity of care across settings.
Qualifications
- Valid state licensure as a Physical Therapist with practicing in a physical therapy role.
- Proven experience in home health settings or outpatient rehabilitation centers, with a strong focus on patient care.
- Knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, and geriatrics to tailor treatments effectively.
- Familiarity with clinical documentation standards and electronic health records (EHR/EMR systems).
- Experience with discharge planning, care plan development, and working with geriatric patients.
- Ability to communicate clearly with patients of diverse backgrounds and collaborate effectively with healthcare teams.
- Prior inpatient or hospital experience is preferred to support comprehensive patient rehabilitation efforts.
