Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) specializing in Long Term Care (LTC) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive nursing care to residents in a long-term care setting, ensuring their safety, comfort, and well-being. The ideal candidate will possess extensive clinical expertise, including experience with complex medical conditions, diverse patient populations, and advanced nursing procedures. Your leadership will support quality patient outcomes while adhering to regulatory standards and best practices in elder and chronic care.
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
- Administer medications accurately, including injections, IV infusions, and specialized treatments such as botulinum toxin therapy.
- Manage complex medical conditions such as dementia, memory care needs, acute pain, infectious diseases, and chronic illnesses common in LTC populations.
- Perform advanced procedures including catheterizations, suturing, tube feedings, wound care, ostomy management, and ventilator support.
- Coordinate interdisciplinary care efforts involving physicians, therapists, case managers, and families to optimize patient outcomes.
- Maintain detailed and compliant medical documentation within electronic health records (EHR/Epic/Cerner systems), ensuring adherence to HIPAA regulations.
- Lead emergency response efforts utilizing critical care protocols for trauma or acute medical events; participate in triage and rapid assessment.
- Provide education and support to residents and their families regarding health conditions, medication management, and discharge planning.
- Ensure infection control practices are rigorously followed to prevent healthcare-associated infections.
- Supervise nursing assistants and other staff members to promote high standards of patient care and safety.
Requirements
- Valid RN license with current registration; experience working in long-term care facilities is essential.
- Demonstrated proficiency with electronic health record systems such as Epic or Cerner; familiarity with EMR/EHR management is preferred.
- Extensive clinical experience including LTC settings; additional experience in assisted living or memory care is highly desirable.
- Knowledge of advanced nursing procedures such as IV insertion, phlebotomy, aseptic technique, and laboratory specimen collection.
- Competence in managing complex medication regimens including narcotics administration and pain management strategies.
- Familiarity with specialized treatments such as botulinum toxin therapy or ostomy care is advantageous.
- Strong understanding of medical terminology, anatomy & physiology knowledge, ICD coding (ICD-9/ICD-10), CPT coding for procedures.
- Ability to perform comprehensive patient assessments across diverse populations including geriatrics, individuals with disabilities or developmental delays.
- Excellent communication skills coupled with critical thinking ability; capable of working effectively within a multidisciplinary team environment.
