TPA is seeking a Family Model Provider for a male member in Clarksville. Compensation total is $47,523.70 which includes Room & Board & 3 meals per day.
Family Model Provider: TPA is approved to provide Family Model services to both the DIDD Waiver and ECF/Choices Enrollees. Family Model Providers are needed to provide room and board in their own homes (apartment or house) to one or two people with disabilities. The Family Model Provider will be responsible for providing person-centered direct personal care including activities of daily living such as cooking, bathing, cleaning, medication administration assistance and medical appointments. Providing supports that will help the people supported reach their goals and outcomes as per the PCSP is paramount to the well-being, independence, and growth of the people supported.
Qualifications and Experience 1. Must have at least 1 year experience working with people with intellectual disabilities. 2. Must pass criminal background check 3. Must have a decent owned or rented home or apartment with a dedicated bedroom for the people supported. Must be wheelchair accessible if a person supported uses a wheelchair 4. Must have a reliable vehicle with auto insurance 5. Must complete all required training, included but not limited to RELIAS, Medication Certification, and CPR/FA. 6. Must have/obtain a computer with internet access for documentation purposes.
Fees or Payment for Services We pay a competitive rate that includes overhead, travel, profit and all direct and indirect costs incurred by Family Model Providers. Depending on the funding level of the person supported (level 1-5), average yearly fees paid to Family Model Provider may range from $20,000 to $59,000 per year providing services 24/7 for 365 days. Family Model providers are not TPA employees, they are considered independent contractors and get the Form 1099 instead of a W-2. In other words, we do not withhold taxes. Also note that your payments may be non-taxable. Please consult with your CPA.

About the Person Supported: Family Model Provider must reside in Clarksville.
Member uses a wheelchair, but is able to transfer into a standard passenger vehicle (not a truck) with little to no assistance. His new home will need to have at least one wheelchair accessible ramp for entry and be wheelchair accessible for his living space. Member requires hands-on assistance with ADLs. He uses grab bares to transfer to the toilet and shower but may need assistance with dressing/undressing. Member needs assistance with meal preparation but is able to eat independently. He requires total assistance with transportation to medical appointments, grocery shopping, and community outings. Member enjoys smoking (outside only), loves sports and football, and wants to gamble once per year.
