JOB DESCRIPTION
FIRST SHIFT CMT
POSITION SUMMARY
Administration of certain medications to residents as prescribed for them by their licensed care providers under the direction the Director of Resident Care and Wellness (or their designee) in accordance with state and federal regulations and community policy. This position may also entail other nursing directed activities including provision of personal care services, assistance, support, and supervision for residents, and other designated duties assigned by their supervisor. This is an essential personnel position. Staff are expected to be at work as scheduled despite weather or catastrophic event.
ESSENTIAL FUNCTIONS
· Primary responsibility includes the preparation, administration and documentation of medication administered to individual residents, ensuring accuracy of person receiving medication, preparation, route, dosage, form, and time of delivery of medication
· Views all residents as a whole, thus reporting any concerns within or outside the realm of medications.
· Provides direct resident care and services as needed or as directed by the Nurse.
· Ensures that medication is verified three times prior to its administration. Ensures label on medication contains all documentation required by federal and state legislation, and that label agrees with current order or is flagged with appropriate change order label.
· Observes each resident as they take their medication. Ensures medication is never left unattended or left with the resident, at the bedside. Insures all topical are applied as ordered.
· Documents cause and effect of all PRN medications and regularly scheduled medications if allowed by state and federal regulations. Observes, records and reports unusual signs, symptoms or reactions to the nurse or duly authorized licensed practitioner. Completes an incident report as soon as possible for adverse medication reactions and in the instance of medication errors, as required by the community policies and procedures. Maintains vital signs each shift for three days, post admission, post fall or for the duration of antibiotic treatment.
· Observes records and reports effects of all medications administered to residents. Records appropriate vital signs whether required before, during or after administration of medication. These may include temperature, pulse, respirations, blood pressure, oxygen saturation, blood glucose, or level of discomfort, in accordance with medication orders or parameters, and in accordance with community policies and procedures.
· Documents administration of medication in the Electronic Medication Administration Record (EMAR) and or other appropriate forms as soon as resident is observed taking medication. Documents any instance when medication is not taken or is refused and reports these instances to supervisor when appropriate (i.e. antihypertensive, hypoglycemic, antipsychotic, antidepressant medications, etc.).
· Consults appropriate drug references to verify proper drug indication, dosage, route, purpose, storage and administration guidelines and precautions, as well as interactions and possible side effects of each medication.
· Assists licensed nurse with chart and medication reviews when requested.
· Demonstrates knowledge of common terms, abbreviations, and symbols related to medications and their administration.
· Stores medications in accordance with current federal and state regulations and policies and procedures of the community. Maintains the security of drug storage areas at all times.
· Utilizes appropriate aseptic techniques in the preparation, administration, and storage of medication.
· Administers medications as prescribed by the physician orders.
· Maintains resident information confidential at all times during medication administration.
· Assists residents with oxygen
· Assists with hearing aids, prosthesis, etc. Records these therapies and incorporates them into the individualized service plan, as well as reporting observations to supervisor.
· Administers medication to residents according to the defined medication procedures.
· Aware of prominent medication adverse effects and interactions and monitors residents for such untoward effects and reports them to the nurse immediately.
· Inspects medication for expiration date, contamination, discoloration or defect.
· When a medication assistance plan is in place, supervises residents who self-administer their medications, according to procedure.
· Adheres to infection control policies of the community.
· Maintains resident records in a timely manner, with documentation completed immediately after administration of medication.
· Maintains the cleanliness and organization of the medication cart, and other medication storage areas.
· Reports all resident requests for PRN medication with the nurse for approval to administer.
· Reports any medication error committed or observed to the nurse immediately and completes incident report form.
· Counts controlled substances with off going nurses, assumes responsibility for accuracy of count before accepting keys.
· Ensures compliance and understanding of all regulations regarding resident’s rights a residents’ rights related to medication administration, recognizing the following:
o Maintains confidentiality of all information in accordance with HIPPA and facility guidelines.
o Immediately reports any violations of resident rights, complaints, or grievances made by residents or their families to the administrator.

Minimum Eligibility Requirements:
Has successfully completed a state approved certification program and able to provide copy of certification.
Requires the ability to communicate in English both orally and in writing
Ability to communicate effectively with residents, families, staff, community officials and the public
Must have compassion for and desire to work with the elderly
Must meet all State health requirements
Must be able to perform duties and responsibilities (Essential Job Functions) with or without reasonable accommodation
Be willing to work a flexible schedule including weekends on a rotation basis.
Must have an acceptable criminal background check to include any required registry check and drug screen.

PHYSICAL REQUIREMENTS
· Physically able to do occasional heavy lifting of up to at least 50 lbs., 30 pounds regularly using proper body mechanics without assistance.
· Physical stamina and ability to assist residents as needed and in emergencies. Must be able to work irregular hours under pressure and stress.
· Working up to six (6) hours standing, walking, and moving about without the need to sit down.
· Manual dexterity required to perform all aspects of care and prescribed treatments for residents, including but not limited to preparation of different forms of medication as well as the ability to administer them; also involves manual dexterity to administer personal care and other therapies as prescribed for residents.
· Physically able to bend, reach and work in small areas.
· Physically able to push and pull, equipment and furnishings.
