Dietician

Location: Ovation Communities — Jewish Home & Care Center
Ovation Communities is seeking a Dietician to assume professional responsibility and accountability for the nutritional health of residents in collaboration with the director of Food Services and Nursing Home Administrator.
Key Responsibilities:

Complete and update dietary assessments to identify residents’ nutritional needs and risks.

Conduct interviews with residents, families, and representatives to develop individualized care plans that respect preferences and rights.

Collaborate with the interdisciplinary team to develop, implement, and evaluate care plans.

Attend care conferences and communicate resident progress, concerns, and responses to interventions.

Complete discharge summaries in a timely manner.

Perform nutritional assessments for assisted living residents as needed.

Experience: Two years’ professional work experience in an institutional health care setting
Education: A bachelor's or graduate degree in nutrition or dietetics
Licenses & Certifications: Registered Dietician by the Commission on Dietetic Registration (CDR)
