Duties
- Supervise and support nursing staff, including registered nurses and home health aides, ensuring adherence to clinical policies and procedures.
- Oversee patient care planning, including assessments, medication management, IV infusion protocols, and wound care such as catheterization and tube feeding.
- Coordinate interdisciplinary team activities to develop personalized care plans that meet patients’ medical and psychosocial needs.
- Ensure compliance with all state healthcare regulations, HIPAA privacy standards, and accreditation requirements like NCQA standards.
- Manage documentation using electronic health record (EHR) systems such as Epic or Cerner, maintaining accurate medical records and medical documentation.
- Conduct regular staff evaluations, provide coaching on clinical best practices including infection control, sterile processing, and aseptic technique.
- Facilitate discharge planning, utilization review, and case management to optimize patient outcomes across settings like assisted living facilities, nursing homes, or long-term care environments.
Skills
- Extensive experience working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or behavioral health needs.
- Proficiency in EMR/EHR systems such as Epic or Athenahealth for documentation and care coordination.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.
- Demonstrated leadership skills in supervising teams within acute care settings like ICU or Level I/II trauma centers; hospital medicine experience is highly valued.
- Expertise in critical care procedures including ventilator management, airway management, IV insertion/injection administration, catheter care, phlebotomy and specimen collection/processing.
- Familiarity with state healthcare regulations governing home health services and long-term care regulations.
- Excellent communication skills for managerial communication with staff members while maintaining patient-centered focus; ability to triage effectively during emergencies or urgent situations.
