Community Referral Coordinator (Part-Time)

Schedule: Flexible (with community visits during business hours)

About Us
Senior Pro Services is a growing home health and private-duty care organization dedicated to providing compassionate, high-quality care throughout the Bay Area.
We are seeking an experienced Community Referral Coordinator to help grow our referral network and strengthen relationships with physicians, hospitals, skilled nursing facilities, and other healthcare partners.

Responsibilities
- Build and maintain relationships with physicians, hospitals, skilled nursing facilities, rehabilitation centers, case managers, discharge planners, social workers, and community organizations.
- Conduct regular in-person visits to referral sources throughout the Bay Area.
- Generate new referral opportunities while maintaining existing referral relationships.
- Educate referral partners about our home health and private-duty care services.
- Represent Senior Pro Services at networking events, healthcare events, and community outreach activities.
- Track referral activity and maintain accurate CRM documentation.
- Follow up consistently with referral partners to build long-term relationships.
- Work closely with leadership to develop referral growth strategies.
Qualifications
- Previous experience in Healthcare Marketing, Home Health, Home Care, Hospice, or Healthcare Business Development preferred.
- Existing relationships within the Bay Area healthcare community are highly desirable.
- Excellent communication and relationship-building skills.
- Self-motivated and comfortable working independently.
- Strong organizational and follow-up skills.
- Professional presentation and networking abilities.
- Valid driver's license and reliable transportation.
