Job Summary
Join a specialized geriatric primary and acute care practice in Brooklyn, New York as an experienced Nurse Practitioner — earning $200,000–$250,000 annually in a uniquely blended clinical role spanning multi-comorbidity management, home hospital medicine, and hands-on wound care including debridement. This is a full-time (part-time option available for the right candidate), fully on-site position with an immediate start for a high-acuity NP ready to deliver close, continuous care to older adults and medically complex patients.
What You'll Do
- Provide primary care for a geriatric and multi-comorbidity panel across the continuum of care
- Deliver acute care services including home hospital medicine for high-acuity patients outside traditional inpatient settings
- Perform comprehensive wound care including assessment, treatment planning, ongoing management, and debridement
- Complete thorough histories, physical exams, and diagnostics — develop and adjust treatment plans accordingly
- Prescribe and manage medications including controlled substances per DEA registration
- Coordinate care with the clinical team, specialists, and families to support safe transitions
- Maintain accurate documentation in compliance with Medicare and payer requirements

Qualifications
- Active, unrestricted APRN license in New York required
- FNP, AGPCNP, or AGACNP board certification required
- Current DEA registration required
- Active Medicare credentialing and enrollment required
- 5+ years of experience required — hospital medicine and critical care strongly preferred
- 1–2 years of dedicated wound care experience including debridement required
- Experience with home-based or hospital-at-home care models preferred
- Experience caring for geriatric and multi-comorbidity populations preferred
