We are seeking a compassionate and dedicated Social Worker to join our healthcare team. The ideal candidate will possess a strong commitment to supporting individuals and families through complex social and emotional challenges. Social Services shall provide comprehensive psychosocial assessment, intervention, advocacy, discharge planning support, crisis intervention, resource coordination, and protection of vulnerable populations to facilitate safe, timely, and effective patient transitions across the continuum of care.
Social Workers shall collaborate with physicians, nursing, case management, rehabilitation services, behavioral health, community agencies, and post-acute providers to address psychosocial barriers impacting treatment, recovery, discharge readiness, and patient outcomes.
Social Services will establish standards for practice to ensure timely psychosocial assessment, safe transitions of care, regulatory compliance, patient advocacy, discharge planning, and coordination of community resources in accordance with Centers for Medicare & Medicaid Services (CMS), The Joint Commission (TJC), Illinois Department of Public Health (IDPH), and hospital standards.
Required Education:
- Master’s Degree in Social Work (MSW) from an accredited school of social work.
- Licensed Social Worker (LSW) required upon hire.
- Licensed Clinical Social Worker (LCSW) preferred

- Minimum of one (1) year of social work experience.
- Minimum of one (1) year of crisis intervention and mental health therapy experience. Experience with guardianship, homelessness, substance abuse, and community resource management strongly preferred.
Desirable Experience:
- Minimum of two (2) years of social work experience in a health care setting.
Duties & Responsibilities:
- Perform discharge planning assessments on all patients.
- Provide Crisis Intervention for emergency room patients.
- Complete a routine Assessment Evaluation within 24 hours of admission for all patients unless admitted over the weekend.

Pay: $74,000.00 - $103,000.00 per year
