The Licensed Practical Nurse (LPN) provides practical nursing services within the scope of practice authorized by the State of Florida, in collaboration with and under the direction of a Registered Nurse (RN). The LPN implements nursing interventions, provides patient and family education, and delivers care in accordance with the physician-ordered and RN-established Plan of Care (POC).
The LPN functions as an integral member of the interdisciplinary care team and supports the delivery of safe, effective, and patient-centered home health services in compliance with Agency policies, state and federal regulations, and CHAP accreditation standards.
Essential Duties and Responsibilities
· Provides nursing care assigned by and under the direction of a Registered Nurse
· Receives RN supervision monthly and as needed.
· Administers treatments and medication within LPN scope of practice and as ordered in the Plan of Care
· Maintains visit frequency as established by the supervising RN
· Utilizes aseptic and infection control techniques in accordance with CDC and Agency policy.
· Prepares and maintains equipment and supplies safely and appropriately.
· Performs home safety and environmental assessments during each visit and reports concerns promptly.
· Assesses and reports changes in ADLs and IADLSs to the supervising RN
· Reinforces the Home Health Aide (HHA) Plan of Care when applicable
· Supports patient dignity, privacy, rights and culturally competent care
· Completes timely, accurate and compliant clinical documentation within 24 hours in the electronic medical record (EMR)
· Reports and documents significant changes in patient condition immediately to the supervising RN
· Communicates recommended Plan of Care updates to RN
· Educates patients and families regarding the Plan of Care, treatments and safety measures.
· Maintains weekly communication with the RN case manager regarding patient’s condition and on any medical changes.
· Participates in case conferences and in interdisciplinary team collaboration with the RN.
· Ensures visit rendered are authorized, medically necessary and billable.
· Attends required meeting, training and in-services
· Serves as a preceptor or mentor to newly engages LPN staff when requested
· Participates in the Agency’s Quality Assurance and Performance Improvement (QAPI) program
· Notifies the Agency promptly of emergencies, illness or events impacting assigned caseload.
· Maintains compliance with CHAP, Medicare Conditions of Participation, Confidentiality (HIPAA) and agency polices
· Demonstrate professionalism, accountability, ethical conduct, and cultural sensitivity
· Notifies agencies promptly of emergencies, illness or circumstances affecting care or caseload.
· Complies with mandatory reporting laws and incident reporting requirements
· Maintains ethical behavior and professional boundaries.
QUALIFICATIONS:
- Graduate of an approved Licensed Practical Nurse (LPN) program
- Current, active Florida LPN license in good standing
- Minimum of one (1) year nursing experience; home health experience preferred
- Successful completion of a criminal background check
- Current CPR certification
- Valid Driver’s License and Automobile insurance.
- IV certified or obtain IV certification upon 2 months of hire.
