Responsibilities
- Conduct comprehensive patient assessments and develop individualized, patient-centered plans of care.
- Manage an assigned patient caseload while collaborating with physicians, therapists, and other members of the interdisciplinary healthcare team to ensure coordinated care.
- Perform skilled nursing visits, monitor patient progress, and evaluate the effectiveness of treatment plans.
- Educate patients and their caregivers on medications, disease processes, treatment plans, self-care techniques, and preventive health measures.
- Recognize changes in patient conditions and promptly update the plan of care to address evolving healthcare needs.
- Maintain accurate, timely, and compliant documentation in accordance with agency policies and regulatory standards.
- Serve as a patient advocate by promoting independence, ensuring safety, and supporting positive health outcomes and quality of life.
Experience
- Prior experience working in home health agency hospital medicine or emergency medicine environments is highly valued for this role.
- Demonstrated proficiency in Level I Trauma Center or Level II Trauma Center settings enhances your ability to handle complex cases effectively.
- Experience in infectious disease care, pediatrics—including PICU—and geriatrics provides a well-rounded foundation for diverse patient populations.
- Familiarity with medical-surgical units, outpatient clinics, or post-acute care settings ensures versatility in patient care delivery.
- Knowledge of anatomy, physiology, and medical terminology supports accurate patient assessment and documentation.
- Proven ability to operate EMR/EHR systems efficiently while maintaining meticulous medical records is essential.
- Valid licensure from the State Board of Nursing along with current CPR/First Aid certifications are mandatory requirements for practicing as a Home Health RN.
Required Qualifications
- Current, unrestricted Texas Registered Nurse (RN) license
- Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical setting
- Excellent assessment, communication, and critical thinking skills
- Reliable transportation, valid driver's license, and current auto insurance
- Willingness to travel throughout the assigned coverage area
- Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support up to 50–100 pounds, including assisting with patient positioning, transfers, and moving equipment.
