⭐Join Our Mission: GSSC – Geriatric Support Services Coordinator (Holyoke, MA – Hybrid)⭐
Pay Rate: $24–$26/hour
Location: Holyoke, MA
Schedule: Full-time, 35 hours/week (8am-4pm or 9am-5pm) | Hybrid after 6-month probationary period (up to 2 days remote *as long as employee is in a satisfactory standing*)
Are you passionate about making a difference in the lives of older adults? Join Access Care Partners as a Geriatric Support Service Coordinator (GSSC) and become an essential part of an interdisciplinary older adult care team dedicated to improving lives and supporting independence.
**BILINGUAL SPANISH/ENGLISH strongly preferred**
Key Responsibilities:
✅ Serve as a vital member of the Primary Care Team (PCT) alongside physicians, RNs, and other care providers
✅ Conduct assessments to determine eligibility and needs for long-term care services
✅ Develop individualized, community-based care plans to support health and independence
✅ Coordinate and authorize services such as home-delivered meals, transportation, and housing support
✅ Track transitions of care and update service plans accordingly
✅ Monitor service quality and intervene when necessary to ensure the best outcomes
✅ Maintain accurate and timely records in the Centralized Enrollee Record system
✅ Advocate for elders’ needs and identify gaps in available services
✅ Collaborate with institutional and community providers to maximize resources
What We’re Looking For
- Bachelor’s degree in a relevant social service field (required)
- Experience or strong interest in elder/human services if degree is in a different field
- Strong organizational, communication, and documentation skills
- Ability to learn and work across multiple database systems
- Knowledge of or willingness to learn federal/state elder service regulations
- Ability to prioritize, work independently, and manage sensitive information
- Experience working with diverse populations and personality types
Additional Requirements
- Valid driver’s license and access to a registered and inspected vehicle
- Ability to conduct in-person assessments
- Satisfactory background check including CORI, SORI, SAMs, OFAC, OIG, and more
- Willingness to travel locally as part of your daily responsibilities
- Comfortable working in a dynamic, evolving care environment
