Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; document findings accurately using Electronic Health Record (EHR) systems.
- Administer medications following prescribed schedules and medical terminology guidelines; ensure proper medication documentation in compliance with home healthcare regulations.
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and Alzheimer's-specific activities.
- Support clients with catheter care, hospice needs, end-of-life care, and working with individuals with developmental disabilities or disabilities.
- Assist with medical procedures such as wound care, catheter management, and basic life support (BLS) or CPR when necessary.
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being.
- Use EMR systems to accurately record patient care activities and medical documentation; adhere to HIPAA guidelines to protect patient confidentiality.
- Support clients' emotional needs through compassionate communication and engagement; work within home & community care settings to foster independence.
Qualifications
- Valid CNA (Certified Nursing Assistant) or PCA (Personal Care Assistant) certification required; prior experience in assisted living or nursing home settings preferred.
- Demonstrated experience working with geriatric patients or individuals with disabilities; familiarity with dementia care and Alzheimer's care highly valued.
- Knowledge of medical terminology, medication administration protocols, catheter care, hospice care procedures, and vital signs monitoring.
- Strong understanding of home healthcare regulations and compliance standards such as HIPAA; experience with EHR systems is a plus.
- Ability to perform heavy lifting safely and handle physically demanding tasks such as Hoyer lift operation when necessary.
- Excellent patient observation skills coupled with caregiving expertise; compassionate attitude towards patients' emotional and physical needs.
- Basic first aid certification and current CPR/BLS certification required; commitment to ongoing professional development in geriatrics and home health practices.
- Reliable transportation and flexibility to work live-in employment arrangements or varied shifts within the home health environment.
