Position Summary
KZL Agency is seeking a compassionate, organized, and motivated Community Living Specialist to oversee services for individuals in our Supported Living and LifeSharing programs. This position coordinates person-centered services, supports providers and staff, monitors compliance, and helps ensure individuals receive high-quality care while living as independently as possible.
Key Responsibilities
- Coordinate services for individuals in Supported Living and LifeSharing.
- Conduct regular home visits to monitor health, safety, and quality of care.
- Support and coach LifeSharing Providers and Direct Support Professionals.
- Participate in ISP meetings and assist individuals in achieving personal goals.
- Maintain accurate documentation and ensure compliance with PA ODP regulations.
- Communicate effectively with individuals, families, Support Coordinators, and interdisciplinary teams.
- Assist with incident follow-up, health and safety monitoring, and problem resolution.
- Participate in an on-call rotation as scheduled.
Qualifications
Applicants must meet one of the following:
- Master's degree and 1 year of experience supporting individuals with intellectual or developmental disabilities.
- Bachelor's degree and 2 years of related experience.
- Associate's degree (or 60 college credits) and 4 years of related experience.
- High school diploma/GED and 6 years of related experience.
Additional Requirements:
- Valid Pennsylvania driver's license.
- Reliable transportation with current insurance.
- Ability to complete all required state and agency training.

Pay: From $50,000.00 per year
