BrightStar Care of Flower Mound, Grapevine, Southlake & Keller is a family-owned healthcare company providing Home Care, Home Health, and Healthcare Staffing services throughout Northeast Tarrant County. As our organization continues to grow, we're looking for an experienced Home Health & Home Care Community Liaison to build referral relationships, develop strategic partnerships, and help more families access exceptional care.
This is an outside business development position for someone who enjoys meeting people, building lasting professional relationships, creating new opportunities, and making a meaningful difference in the lives of patients and families. Compensation & Benefits
- $50,000 annual base salary plus performance bonus
- Typical annual earnings: $75,000-$125,000+

What Success Looks Like
Success in this role means consistently generating qualified referrals, expanding strategic partnerships, increasing market share throughout Grapevine, Southlake, Keller, and surrounding communities, and representing BrightStar Care as the premier provider of Home Care and Home Health services. This role is ideal for someone who enjoys building relationships, identifying opportunities, and taking ownership of growing a defined territory with minimal supervision.
Primary Responsibilities
Grow Personal Care Services
- Build and strengthen relationships with senior living communities, placement agencies, long-term care insurance providers, veterans organizations, and community partners.
- Prospect, develop, and strengthen relationships with new and existing referral partners throughout the territory.
- Conduct in-home consultations with prospective clients and families to understand their needs and recommend appropriate care solutions.
- Represent BrightStar Care at community events, educational seminars, networking functions, and health fairs.
- Support client onboarding, conversion, and retention initiatives.
- Help strengthen BrightStar Care's reputation through community engagement and online reviews.
Grow Home Health Services
- Develop relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, specialty clinics, case managers, discharge planners, and other healthcare professionals.
- Identify new referral opportunities and strategic partnerships.
- Collaborate with clinical leadership to improve referral conversion and customer satisfaction.
- Maintain regular communication with major referral partners to support long-term growth.
- Represent BrightStar Care at healthcare networking events and professional associations.
Territory Management
- Develop and execute strategic territory growth plans.
- Maintain a disciplined weekly schedule of prospecting, relationship management, and follow-up.
- Maintain accurate CRM records, referral activity, and pipeline management.
- Collaborate with Operations, Clinical Leadership, and Intake teams to support agency growth.
- Contribute to agency growth planning and strategic business development initiatives.
- Provide support to fellow Community Liaisons when needed.
Qualifications
We're looking for someone who has:
- 4+ years of healthcare business development, physician liaison, home health, hospice, home care, pharmaceutical, medical device, or related outside sales experience
- Demonstrated success developing referral relationships and growing a sales territory
- Excellent communication, networking, and relationship-building skills
- Experience working with senior living communities, hospitals, physician practices, or case management teams
- Strong organizational and time-management abilities
- Confidence asking for business and closing opportunities
- Ability to work independently with minimal supervision
- Experience using Microsoft Office and CRM software
- Valid driver's license, reliable transportation, and proof of automobile insurance
Preferred Qualifications
- Home Health or Home Care marketing experience
- RN or LVN license
- Existing relationships within Grapevine, Southlake, Keller, Colleyville, HEB, or surrounding healthcare communities
- Bachelor's degree
