Fee-for-Service UAS Assessment Nurse (RN) – In-Person | NY Metro Area

We are currently seeking Fee-for-Service UAS Assessment Nurses (RNs) to conduct in-person assessments across New York. These evaluations are performed on behalf of select Managed Long-Term Care (MLTC) plans and waiver case assessments.
Job Responsibilities
- Perform comprehensive in-home MLTC assessments
- Determine member eligibility for MLTC programs
- Develop and update Person-Centered Service Plans
- Review, assign, and manage incoming assessment requests
- Track and submit assessments in compliance with NYS and agency regulations
- Conduct in-person evaluations using the UAS tool (functional, medical, behavioral, and social needs)
- Educate members on self-management of medical conditions
- Maintain accurate case records, including medication reconciliation
- Conduct monthly outreach to monitor member status
- Enter assessment data into the HCS system accurately in a timely manner
Job Requirements
- Must have 6 months to 1 year of experience as a UAS (Uniform Assessment System) Assessor with MetroPlus
- Bilingual highly preferred: Spanish, Russian, or Chinese (other languages a plus)
- Strong computer proficiency
- Excellent communication and assessment skills
- Highly organized and detail-oriented
- Ability to work independently
- Experience in home care, acute care, sub-acute care, LTC, or MLTC preferred
Applicant Requirements
- Current and valid NYS RN License
- Valid documentation authorizing eligibility to work in the U.S.
- Two professional reference letters
- Physical exam (within the last year)
- Proof of Rubella & Rubeola immunity
- QuantiFERON test (chest X-ray required if positive)
- Professional Liability/Malpractice Insurance
- Direct deposit form or voided check (optional)
Compensation & Benefits
- $120 – $150 per in-person assessment
