MEMPHIS / JACKSON, TN — Nurse Practitioner, In-Home Wound Care
$150,000 salary. Monday through Friday, no weekends. A manageable panel averaging 5 to 6 patient visits per day, clustered by city to reduce drive time.
Our client, Esperta Health, is a physician-led, in-home wound care company serving patients across West Tennessee, and we are hiring a full-time Nurse Practitioner to join the team.
No clinic. No hospital politics. Just you, your patients, and a proven care model backed by strong physician leadership.
You will deliver wound care directly in patients' homes across West Tennessee. Your days are clustered by city, meaning full days in Memphis or in Jackson rather than crossing the region on the same day, so you get a steady, manageable schedule without constant windshield time. The operation runs on a proven care model and infrastructure refined across multiple markets, so your focus is patient care from day one. You manage your own schedule and territory with real autonomy, supported by physician leadership.
What You Will Do
Provide in-home wound care to patients with complex wounds across West Tennessee, averaging 5 to 6 visits per day, with days clustered by city. Perform wound assessments, debridement, and treatment planning. Identify root causes of non-healing including pressure, perfusion, and nutrition issues. Collaborate with physicians on care plans. Work alongside skilled nursing facilities and referring providers involved in your patients' care. Depending on the market, you may have RN or LPN support, with room to take on more clinical leadership as your panel grows.
What We Are Looking For
Active, unrestricted Nurse Practitioner license in Tennessee (board certified). At least 3 years of wound care experience. Mobile wound care, home health, or wound care clinic background preferred. Comfortable working independently in patients' homes. Willingness to cover both the Memphis and Jackson markets, with days clustered by city. Valid driver's license.
