Description
Provides education as well as oversight and acts as a liaison with the care transitions team, patients, physicians, families, other health care personnel, hospital administration, external agencies and community groups. Ensures best practices for the quality of transitional care services. Responsible for performing job duties in accordance with mission, vision, and values of Tampa General Hospital.
Essential Functions:

Plan and maintain the schedule and assignments for the team.

Complete weekly timecards and payroll.

Prepares and facilitates daily interdisciplinary huddles/reviews to improve and maintain quality patient outcomes within specified populations and programs.

Planning and oversight of the orientation of new team members.

Contributes to the performance evaluation process.

Monitors business operations and productivity.

Responsible for process improvement, care coordination, Remote Patient Monitoring (RPM) and Transitions of Care.

Monitors program metrics and is responsible for clinical supervision and oversight for the Care Transitions team.

Supports and guides Care Transitions team members and their patients in the health care decision-making process including but not limited to admissions, diagnostics, surgical/medical interventions, discharge, and post-acute support services.

Patient care needs will vary depending on the service line growth and strategy.
Qualifications

Bachelor's Degree Nursing

Active Florida Nursing License
-
3 years related work experience required; BSN may be completed within 2 years of hire from an accredited institution
