Job Summary
As a Personal Care Aide or Home Health Aide, you will provide essential support to clients in their homes and community settings. Your role is vital in promoting independence, dignity, and quality of life. You will follow individualized care plans, deliver hands‑on assistance, and maintain a safe, respectful environment for every client you serve.
Responsibilities
- Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, mobility
- Support safe transfers, including use of Hoyer lifts and other assistive devices
- Monitor and document client condition, including vital signs and changes in health
- Follow care plans and provide medication reminders per guidelines
- Prepare meals and assist with feeding as needed
- Maintain a clean, safe environment using proper infection‑control practices
- Provide companionship, emotional support, and social engagement
- Accurately document care provided, observations, and incidents
Qualifications
- PCA or HHA certification (or willingness to train)
- Experience in home care, assisted living, group homes, or long‑term care preferred
- Knowledge of dementia care, Alzheimer’s care, autism support, hospice, or developmental disabilities is a plus
- Ability to lift and assist with transfers safely
- Strong communication skills and ability to follow detailed care plans
- Compassionate, patient, and committed to providing person‑centered care
