Job Title: Psychosocial Rehabilitation (PSR) Provider

Purpose of the Position:
The PSR role provides rehabilitative services that aim to enhance the social, emotional, and functional skills of
children/youth, fostering independence and promoting integration within family and community settings.
Reports To: Assigned CFTSS Supervisor
Supervises: Not Applicable
1. Specific Responsibilities
- Engage children/youth in activities that develop social, emotional, and daily living skills to improve functioning
within the community.
- Implement structured interventions that support independence, enhance interpersonal skills, and promote
positive behaviors.
- Educate and support families in reinforcing these skills at home and in the community.
- Facilitate the development and monitoring of individualized service plans with clear, achievable goals.
- Attend monthly waiver service provider meetings to ensure consistency in service delivery and stay informed on
updated practices and protocols.
- Document services provided, including progress notes, service plans, expense logs, and other required records,
submitted within prescribed timeframes.
- Report any incidents or concerns immediately to the supervisor to maintain client safety and service
transparency.
- Coordinate with other service providers and the Care Manager to discuss progress, address challenges, and
ensure continuity of care.
- Assess the suitability of community or home settings for providing services, tailoring interventions to meet the
child/youth’s needs.
2. Qualifications
- At a minimum, staff must be 18 years old and have a high school diploma, high school equivalency preferred, or
a State Education Commencement Credential with a minimum of two years’ experience in children’s mental
health, addiction, foster care and/or a related human services field; or
- A bachelor’s degree in social work, psychology, or in related human services; or
- A master’s degree in social work, psychology, or in related human services
- Experience in rehabilitative or human services, particularly with children/youth.

Pay: $30.00 - $35.00 per hour
