American Traveler is hiring an experienced RN Care Manager for an acute care case management role requiring 2+ years of RN experience and NC or compact licensure.

Acute care hospital setting within a 20-25 bed unit covering Cardiology, Nephrology, Trauma surgery, Ortho surgery, Vascular surgery, Neurosurgery, GI surgery, Urology, and Transplant, including ICU/Stepdown for those services

Manages acutely ill patients with complex and varying medical needs
-
13-week assignment with a schedule of 40 hours per week, 8 hours per day, 5 days per week
-
Day shift with non-standard hours from 8:30 AM to 5:00 PM
-
No weekend, holiday, or on-call requirements

Uses Epic EMR for documentation and care planning

Active RN license in North Carolina (Standard or Compact)

Minimum of 2 years of healthcare experience as a Registered Nurse
-
Prior experience as a case manager, care manager, or discharge planner in an acute care hospital setting required

Nursing Diploma, ADN, or ASN from an accredited nursing school required; BSN or MSN preferred

Strong assessment and critical thinking skills

Epic EMR experience preferred

Experience in educational or teaching hospital settings is helpful

Conduct comprehensive patient assessments and document plans of care in Epic per departmental guidelines
-
Prioritize daily patient caseload and identify new admissions through work list review
-
Participate in Daily Care Management Touchpoints and consult with Social Work per established criteria
-
Attend and actively participate in CAPP meetings to discuss patient progression and discharge readiness
-
Attend weekly Complex Care Meetings to collaborate on complex patient cases and identify trends
-
Coordinate with multidisciplinary team members to address barriers to discharge and psychosocial concerns
-
Facilitate family meetings and provide education on community resources and post-acute care options
-
Monitor observation patients and assess ongoing discharge readiness in collaboration with patient, family, and care team

Identify required authorizations for post-discharge services and coordinate referrals to post-acute providers
-
Refer administrative tasks to Care Management Assistant as appropriate

Works within a collaborative care team structure including Social Workers, Utilization Managers, and Care Management Assistants
