Position Summary
PurposeCare is seeking an experienced Assistant Director of Nursing (ADON) to support the clinical operations of our Home Health agency. Reporting directly to the Admin/ Director of Nursing, the ADON plays a key leadership role in ensuring quality patient care, regulatory compliance, clinical staff development, and operational excellence.
The ADON serves as a clinical resource for field staff, assists with oversight of case management, OASIS quality, documentation review, and performance improvement initiatives while helping maintain compliance with Medicare Conditions of Participation (CoPs) and agency policies.
This position is ideal for an experienced Home Health RN who is ready to expand their leadership career while continuing to make a direct impact on patient outcomes.
Essential ResponsibilitiesClinical Operations
- Support the Clinical Director in the oversight of daily clinical operations.
- Provide leadership and guidance to Registered Nurses, Licensed Practical Nurses, therapists, and interdisciplinary team members.
- Assist with clinical decision-making, patient care coordination, and complex case management.
- Monitor patient outcomes and ensure high-quality, patient-centered care.
OASIS & Clinical Documentation
- Review and audit OASIS assessments for accuracy, completeness, and regulatory compliance.
- Monitor documentation quality and provide coaching to clinicians on best practices.
- Assist with coding accuracy and documentation improvement initiatives.
- Ensure timely completion of physician orders, care plans, and clinical documentation.
Quality & Compliance
- Assist with Quality Assurance and Performance Improvement (QAPI) initiatives.
- Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, accreditation standards, and agency policies.
- Participate in survey readiness, regulatory audits, and corrective action planning.
- Monitor infection prevention, patient safety, and risk management activities.
Staff Leadership & Development
- Mentor, coach, and develop clinical staff through education and ongoing support.
- Assist with orientation, competency validation, and annual clinical education.
- Support employee engagement, performance improvement, and retention initiatives.
- Serve as a clinical resource for field staff regarding patient care and regulatory questions.
Collaboration & Agency Support
- Partner with the Clinical Director, Administrator, Intake, Scheduling, Therapy, and Business Development teams to ensure seamless patient care.
- Participate in case conferences and interdisciplinary team meetings.
- Assist with staffing coordination and workload management.
- Support agency growth by maintaining high-quality clinical outcomes and exceptional patient experiences.
Qualifications
- Active Registered Nurse (RN) license in good standing.
- Minimum of 3 years of Home Health experience required.
- Previous experience as a Clinical Manager, Patient Care Manager, Assistant Director of Nursing, Clinical Supervisor, or similar leadership role preferred.
- Strong understanding of Medicare Conditions of Participation (CoPs), OASIS, case management, and home health regulations.
- Experience with EMR systems such as Matrix, Continulink, Alayacare, Axxess, Homecare Homebase (HCHB), WellSky, or similar platforms preferred.
- Demonstrated ability to mentor clinicians, manage priorities, and lead in a collaborative environment.
- Excellent communication, critical thinking, organizational, and leadership skills.
