Job Summary
We are seeking a dynamic and compassionate Case Manager to join our dedicated social services team. In this role you will work with a team of other case managers, help with benefits for clients, have a working knowledge of working in addiction and dual diagnosis treatment. You will also have experience in setting up clients with services, appointments, discussing recovery and the 12 steps/other recovery programs, facilitate groups, make family contact calls, and work on Utilization Reviews.
Responsibilities
- Work with a team of case managers and social service staff, fostering a collaborative and supportive work environment.
- Develop, implement, and evaluate individualized care plans tailored to clients’ needs, ensuring compliance with all regulatory standards.
- Manage program operations, including budgeting, documentation, reporting, and maintaining accurate client records using Microsoft Office tools.
- Build and maintain strong relationships with clients, families, community partners, and service providers to facilitate seamless service delivery.
- Coordinate behavioral health interventions and support services that promote client independence and community integration.
- Help with benefits for clients, have a working knowledge of working in addiction and dual diagnosis treatment. Experience
- Proven experience working with individuals in addiction and mental health.
- Demonstrated leadership in social services administration with supervisory experience managing teams of program staff or case managers.
- Strong relationship management skills with the ability to communicate effectively with diverse populations, including bilingual clients when applicable.
- Administrative expertise in program development, budgeting, compliance monitoring, and documentation practices.
- Experience working with underserved communities, understanding their unique needs and barriers to access.
- Familiarity with Microsoft Office applications for report writing, data tracking, and communication purposes.
- Knowledge of behavioral health principles and best practices in client-centered care planning.

Pay: $22.50 - $26.00 per hour
