Join our dynamic home health team as a dedicated Speech Therapist working 3 days a week to provide expert speech and language therapy services to patient diverse populations, including pediatrics and geriatrics, within the comfort of patients' homes. Duties
- Conduct thorough speech and language evaluations for patients across various age groups, including children with developmental delays and adults with speech impairments.
- Develop, implement, and monitor individualized speech-language therapy plans tailored to each patient's unique needs.
- Deliver engaging therapy sessions via in-person visits or telehealth platforms, utilizing evidence-based techniques to promote communication improvement.
- Collaborate closely with multidisciplinary teams—including physicians, occupational therapists, physical therapists, educators, and families—to ensure cohesive patient care strategies.
- Educate and empower parents, caregivers, and patients by providing clear guidance on speech-language exercises and strategies for ongoing progress at home.
- Perform detailed patient assessments focusing on physiology and anatomy to inform accurate diagnoses and effective treatment approaches.
- Maintain precise documentation of therapy sessions, progress notes, and evaluations within Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems in compliance with healthcare regulations.
Qualifications
- Proven experience working with pediatric populations in early intervention or school settings is highly desirable.
- Strong knowledge of speech-language pathology principles, including comprehensive speech and language evaluations and therapy plan implementation.
- Familiarity with telehealth therapy platforms and experience delivering remote speech-language services.
- Ability to work effectively with individuals with disabilities or speech impairments across diverse age groups.
- Excellent communication skills for parent collaboration and multidisciplinary teamwork.
- Knowledge of medical terminology, anatomy, physiology related to speech and language functions.
- Prior experience working with geriatric patients in home health or similar settings is a plus.
- Valid licensure or certification as a Speech-Language Pathologist (SLP) or related credential required; RN licensure preferred if applicable to the role.
