We are seeking a compassionate and dedicated Nurse to join our healthcare team. The ideal candidate will provide essential support to patients in the comfort of their home. This role requires a strong commitment to patient care, excellent communication skills, and the ability to work effectively in a one on one environment. Oasis experience is a plus
Responsibilities
- Educate patients on disease process, avoid hospitalization
- Monitor and record vital signs, including temperature, blood pressure, and pulse.
- Provide support in administering medications and treatments
- Maintain accurate medical documentation and assist with electronic health records (EHR) management.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
- Wound Care, IV therapy
- Support patients with special needs, including those with developmental disabilities, dementia, or Alzheimer’s care.
Requirements
- Certification as a Registered Nurse or Licensed Practical Nurse in a healthcare setting is preferred.
- Experience in critical care, long-term care, assisted living, or home care is highly desirable.
- Familiarity with EMR systems such as Axxess.
- CPR certified
- Strong interpersonal skills with the ability to work effectively with patients of all ages and backgrounds.
- Experience in hospice care, geriatrics, pediatrics, dementia or behavioral health is beneficial.
- Ability to perform tasks related to patient service and caregiving while maintaining HIPAA compliance.
