Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions regularly, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications and treatments as prescribed, following all home healthcare regulations and safety protocols
- Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates to track patient health trends
- Support patients with specialized needs such as Alzheimer's care, dementia care, hospice care, or end-of-life assistance with sensitivity and professionalism
- Assist with medical procedures including catheter care, wound dressing changes, and patient transfer methods using equipment like Hoyer lifts or other assistive devices
- Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe living environment
- Maintain strict confidentiality of patient information in compliance with HIPAA regulations while communicating effectively with clients and healthcare teams
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred
- Demonstrated knowledge of medical terminology and familiarity with home health documentation practices
- Experience working with individuals with developmental disabilities or geriatrics is advantageous
- Certification in First Aid and Basic Life Support (BLS), including CPR training required; additional certifications such as hospice or dementia care are a plus
- Proven ability to handle heavy lifting safely and operate patient transfer equipment like Hoyer lifts or similar devices
- Knowledge of home health agency operations and familiarity with EMR/EHR systems for accurate medical records management
- Strong communication skills to effectively engage with patients, families, and healthcare professionals while maintaining patient confidentiality
