Home Health Therapist - PT | OT | PTA | COTA | SLP
Compensation: $60-$120 per visit (based on discipline and experience)
We're seeking licensed therapists to join our growing Medicare Part A Home Health team. This position offers the flexibility to build a schedule around your availability while providing high-quality, one-on-one care to patients in their homes.
Coverage Area
Including the following areas:
Hawthorne, Lawndale, Gardena, South Gate, Compton
Responsibilities
- Provide skilled therapy services in the home setting under Medicare Part A.
- Evaluate patients and establish individualized plans of care (PT/OT/SLP).
- Deliver treatment focused on improving mobility, independence, safety, and quality of life.
- Collaborate with physicians, nursing, and the interdisciplinary care team to coordinate patient care.
- Complete accurate, timely documentation using our electronic medical record.
- Manage your assigned geographic territory while maintaining excellent patient outcomes.

Qualifications
- Current state license as a PT, OT, PTA, COTA, or SLP.
- Current driver's license, reliable transportation, and auto insurance.
- Strong communication and clinical decision-making skills.
- Home health experience is preferred but not required.
