American Traveler is hiring an experienced RN Care Manager for a hospital position requiring 5+ years of experience and a ME or compact RN license.

Work within the Care Management department of a hospital setting

Patient ratio of approximately 1:20-24
-
Day shift schedule, Monday through Friday, 8:00 AM to 4:30 PM
-
Every third weekend required

Holiday coverage required
-
No on-call responsibilities

Floating required to cover Care Management department needs as well as other facilities within a 25-mile radius

Uses Epic EMR
-
Active RN license (ME or Compact) required
-
5+ years of experience required
-
Experience in discharge planning and care coordination required

Driver's license required for consideration

Two professional references required

Responsible for discharge planning and care coordination for assigned patients
-
Ideal candidate is flexible, easygoing, and a calm problem solver with a can-do attitude

Scrubs or professional casual dress is acceptable attire

Candidates residing within 90 miles of any MaineHealth facility (Maine or New Hampshire residents) will be considered local, with a corresponding adjustment to the travel rate

Local travelers are not eligible for contract renewals or extensions

RTO should be kept to a minimum, with 3-4 days considered reasonable for a 13-week contract at the manager's discretion
