ABOUT THE ROLE
The Registered Nurse plans, organizes, and directs home health services for assigned patients. The RN completes comprehensive assessments, develops and updates individualized plans of care, provides skilled nursing services, coordinates care across the clinical team, and supports patients and families throughout the episode of care. The right candidate is clinically strong, self-directed, organized, and committed to safe, compassionate care with complete and timely documentation.
RESPONSIBILITIES
Complete initial and ongoing assessments of patients and families to determine home health needs.
Perform comprehensive physical assessments and obtain relevant current and prior health histories.
Initiate, maintain, and revise the plan of care as the patient's condition and needs change.
Develop measurable care goals that include therapeutic, preventive, and rehabilitative nursing interventions.
Include the patient and family in the care-planning process.
Provide skilled nursing services within the RN scope of practice and according to physician orders and the plan of care.
Administer medications and treatments as ordered.
Provide patient and caregiver education based on the assessment and plan of care.
Identify discharge-planning needs and implement an appropriate discharge plan.
Serve as Case Manager when assigned and coordinate care for the designated caseload.
Communicate promptly with physicians regarding patient needs, changes in condition, and required orders.
Coordinate with therapists, aides, community providers, vendors, and other members of the care team.
Ensure required equipment, supplies, and support services are available.
Prepare complete, accurate, and timely clinical notes.
Participate in on-call responsibilities as assigned under agency policy.
Instruct, supervise, and evaluate home health aide services at required intervals.
Follow agency policies, infection-control practices, professional standards, and documentation requirements.
Report safety concerns, incidents, changes in condition, and potential compliance issues promptly.
QUALIFICATIONS
Current California Registered Nurse license in good standing.
Graduate of an accredited school of nursing.
Current CPR certification.
Minimum of two years of nursing experience preferred, including at least one year in home health, community health, or a related setting.
Strong assessment, clinical judgment, observation, problem-solving, and case-management skills.
Excellent verbal and written communication.
Ability to organize and prioritize an independent field workload.
Ability to work with minimal direct supervision while maintaining communication with the clinical team.
Valid driver's license, reliable insured vehicle, and ability to travel to patient homes.
Ability to perform the physical functions required for home-based patient care, including walking, standing, reaching, bending, positioning, and transferring patients and equipment.
