About the Role
Select Home Health & Hospice is seeking an Intake Coordinator to support the referral and admission process for Home Health and Hospice patients. In this role, you will manage patient referrals, verify insurance coverage, maintain accurate patient and insurance records, and coordinate communication between referral sources, patients, and internal teams. The ideal candidate is detail-oriented, organized, and comfortable working in a fast-paced environment.
Responsibilities

Receive and process referrals for Home Health and Hospice services from hospitals, physicians, facilities, and other sources.

Accurately enter and update patient demographic, insurance, and clinical information in EMR systems.

Upload, organize, and manage referral and admission documentation in compliance with agency procedures.

Verify insurance coverage and obtain authorizations when required; communicate any coverage or eligibility issues.

Ensure all admission documentation is complete, accurate, and compliant prior to the start of care.

Support clinical and operational review of referrals to confirm appropriateness of services.

Maintain ongoing communication with referral sources, patients, and internal teams to ensure timely admissions.

Track pending referrals and follow up to ensure timely review, approval, and admission or non-admit decisions.

Escalate concerns related to staffing, service capability, or patient appropriateness to leadership.

Maintain patient confidentiality in accordance with HIPAA and agency policies.

Participate in staff meetings, training, and quality improvement initiatives as assigned.
Physical Requirements

Prolonged periods of sitting while working at a desk and using a computer.

Frequent use of hands and fingers to operate a computer, keyboard, and telephone.

Regular ability to communicate effectively, including speaking and hearing.

High school diploma or equivalent (required); associate or bachelor’s degree in a healthcare-related field (preferred)

Minimum of 2–3 years of experience in home health and/or hospice intake, admissions, or care coordination strongly (preferred)

Experience with insurance verification, authorizations, and EMR systems (required)

Knowledge of home health/hospice regulations and OASIS (preferred)

Ability to work in a fast-paced environment and manage multiple priorities (required)
