This job description, defining qualification, primary and secondary functions, responsibilities and duties is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. The employee in this position performs these and other duties as required although not listed which may be assigned by his supervisor or facility Administrator.
ESSENTIAL FUNCTIONS:
Provides resident treatments as specified by the Occupational Therapist in the treatment plan as listed in the Skills Checklist.
Assists the Occupational Therapist in the provision of direct resident treatments.
Maintains records and documentation pertinent to resident treatments.
Assists Occupational Therapist in preparation of Medicare forms and other required documentation.
Communicates with physicians, families and rehab team staff members regarding resident status reports as appropriate.
Assists the Occupational Therapist in the scheduling of residents.
Participates in resident care conferences, rehabilitation team conferences and represents the rehab department at facility meetings as assigned by the Occupational Therapist.
Assists residents with the preparation for treatment, i.e., applying braces, orthotics and prosthetics.
Involved in the Restorative Feeding and/or Rehab Feeding Program, as assigned by the Occupational Therapist.
Directs and supervises Rehab Aide.
Follows Resident’s Rights policies at all times.

Maintains 75% - 80% productivity level.
Assumes responsibility for reading and following all documentation guidelines in the Documentation Manual.
Transports patients to and from therapy.
Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.
Safety & Health Requirements:
Identifies safety hazards and initiates corrective action.
Follows all facility safety policies and procedures.
Observes infection control procedures standards.
Professional Development Requirements:
Studies current trends and new developments in Occupational therapy treatment by survey of current literature and attendance of professional meetings and conferences.
Consistently demonstrates responsibility, accountability and dependability.
Consistently works cooperatively and maintains effective communication with facility administration, nursing personnel, ancillary personnel, all facility departments and consultants.
Comes to work clean and neat and consistently presents a professional appearance.
Accepts assigned duties in a cooperative manner.
Consistently maintains awareness of changes in rules and regulations effecting facility operations.
NON-ESSENTIAL FUNCTIONS:
Participates in the facility CQI Program.
Performs clerical duties as assigned by the Director of Rehab.

Prerequisites:
Current CPR Certification required.
Associates Degree: Graduate from an accredited program for Certified Occupational Therapist Assistant.
Current registration with American Occupational Therapy Association preferred.
Valid state license in Occupational Therapy necessary.
Licensed eligible necessary.
Experience preferred, but not necessary.
Knowledge of growth and development across the life span required.
