We’re looking for a compassionate and dependable Field Nurse (RN or LPN) to complete in‑home assessments and support our HCBS clients. If you enjoy working independently and making a real impact in your community, this role is a great fit.
Position Highlights
Part‑time or PRN Flexible schedule
Pay rate: $30 per visit Local travel required
What You’ll Do
Conduct in‑home nursing assessments Complete initial, annual, and as‑needed reassessments Monitor client health and report changes Review and update Plans of Care Educate clients, families, and caregivers Ensure LDH & Medicaid compliance Document visits accurately and timely
Requirements
- Active RN or LPN license in Louisiana and Texas
- Experience with Homemaker, HCBS, LT‑PCS, CCW, or waiver programs is a plus
- Strong assessment and documentation skills
- Reliable transportation
- Professional, caring, and dependable
