Job Overview
We are seeking a dedicated and skilled Occupational Therapist (OT) to join our home health team on a PRN (per visit) basis. This role offers flexibility to provide patient-centered care in the comfort of patients’ homes and community settings. The ideal candidate will have a strong OT background with experience across care environments such as outpatient clinics, hospitals, inpatient rehab, and home health. The OT will be responsible for developing and implementing individualized care plans to maximize functional independence and safety for diverse patient populations, including geriatrics, orthopedics, neurology, and patients requiring acute or chronic care.
Duties
- Conduct comprehensive OT evaluations to identify functional limitations and therapy needs
- Develop, implement, and modify individualized plans of care based on patient goals and physician orders
- Provide skilled OT interventions focused on ADLs and IADLs (bathing, dressing, toileting, transfers, meal prep, etc.)
- Address upper extremity function, strengthening, coordination, fine motor skills, and therapeutic activity as appropriate
- Assess home safety and recommend modifications, adaptive equipment, and fall prevention strategies
- Educate patients and caregivers on safe techniques, energy conservation, and home programs
- Coordinate discharge planning and communicate progress with the care team
- Complete accurate, timely documentation in the EMR in compliance with applicable regulations and payer requirements
- Support care transitions between inpatient, outpatient, and home health settings as needed
- Collaborate with physicians, nurses, PTs, STs, and other healthcare professionals to deliver coordinated care
Qualifications
- Valid state licensure as an Occupational Therapist (OT)
- Home health / community-based experience strongly preferred
- Solid understanding of anatomy, physiology, medical terminology, and OT evaluation and treatment techniques
- Experience in acute care, inpatient rehab/hospital, outpatient, or geriatrics is a plus
- Familiarity with EMR systems for documentation and patient management
- Knowledge of functional care planning across orthopedics, geriatrics, neuro, and general patient care needs
- Strong communication skills for patient/caregiver education and interdisciplinary coordination
- Ability to work independently in home settings while maintaining professional standards and timelines
