Position: Remote CCM Manager
Summary:
Seeking an experienced, organized LVN (home health experience preferred) to remotely manage Medicare Chronic Care Management patients for a busy cardiology practice. The CCM Manager will provide monthly patient outreach, coordinate care, identify clinical needs, document billable time, and improve patient outcomes.
Responsibilities:
- Complete monthly CCM phone calls with assigned patients.
- Assess symptoms, medication adherence, and barriers to care.
- Coordinate care with providers, specialists, pharmacies, and community resources.
- Educate patients on chronic disease management and preventive care.
- Identify changes in condition requiring provider review.
- Accurately document all CCM activities and billable time in the EMR.
- Ensure compliance with CMS CCM requirements.
- Assist with medication reconciliation and follow-up after hospital or ER visits.
- Maintain excellent patient satisfaction through compassionate communication.
Qualifications:
- Active LVN license.
- Home health, case management, or chronic disease management experience preferred.
- Strong telephone communication and patient education skills.
- Comfortable working independently from home.
- Experience with EMRs (Athena preferred).
- Excellent organizational and documentation skills.
Pay: $18.00 per hour
