Description:
The Chaplain provides a wide range of spiritual services to patients and families. Additionally, the Chaplain offers spiritual support to the staff and volunteers of the Company.
Job Responsibilities:
- Develop spiritual care plans based on patient/family spiritual traditions and assessment results.
- Provide appropriate spiritual care and initiate interventions to resolve care plan issues.
- Implement spiritual care plans through personal contact or by enlisting volunteers or local clergy.
- Monitor plan implementation and reassess as needed.
- Maintain timely and accurate documentation of care provided.
- Act as a liaison between the Company and local clergy and congregations.
- Attend interdisciplinary team meetings, contribute to care plans, and provide spiritual care objectives.
- Perform other duties as assigned.
Qualifications:
- Bachelor’s degree in divinity or related field from an accredited seminary or institution; master’s degree preferred.
- Ordination or commissioning/endorsement from a recognized ordaining body
- Documentation demonstrating good standing with faith group or professional affiliation.
- Completion of one unit of Clinical Pastoral Education (CPE) accredited by the Association for Clinical Pastoral Education, Inc., or in process within six (6) months of employment.
- Previous experience working with an EMR/EHR system preferred.
- Health care experience preferred.
- Valid driver's license and automobile insurance per Company policy
- Ability to travel and have reliable transportation.
- Must be able to navigate various environments, including homes, hospitals, nursing homes, and assisted living facilities.
- Excellent communication skills in English, bilingual in Spanish preferred.
