Responsibilities
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support.
- Monitor and record vital signs, medication administration, and changes in patient condition accurately using Wellsky.
- Support patients with medication management while adhering to HIPAA regulations to ensure confidentiality and privacy.
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including engaging in therapeutic activities and ensuring safety.
- Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met.
- Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams to ensure continuity of care.
Qualifications
- Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
- Proven experience working in assisted living facilities, nursing homes, or home health environments.
- Strong knowledge of medical terminology, patient care procedures, and documentation practices.
- Experience working with individuals with developmental disabilities or special needs is highly desirable.
- Ability to perform patient observation accurately and respond promptly to changing conditions.
- Certifications in CPR and first aid are required; additional training in hospice or dementia care is a plus.
- Familiarity with EMR/EHR systems for medical documentation and record keeping.
- Demonstrated ability to work independently while maintaining excellent communication within multidisciplinary teams.

Pay: $16.00 - $20.00 per hour
