About the Role
We are seeking compassionate and motivated Physical Therapists to provide skilled home health services throughout the Bay Area. In this role, you will evaluate and treat patients recovering from illness, injury, surgery, or functional decline while helping them improve mobility, strength, balance, safety, and independence in their home environment.
You will work closely with physicians, nurses, caregivers, and interdisciplinary team members to deliver coordinated, patient-centered care and help patients achieve their rehabilitation goals.
Qualifications
- Current California Physical Therapist License required
- Valid California Driver's License required
- Strong assessment, treatment planning, and documentation skills
- Excellent verbal and written communication abilities
- Ability to work independently while collaborating with an interdisciplinary team
- Home health experience preferred but not required
- New graduates welcome to apply
Key Responsibilities
- Conduct comprehensive patient evaluations in the home setting
- Develop individualized treatment plans based on physician orders and clinical findings
- Deliver skilled physical therapy interventions utilizing evidence-based treatment approaches
- Monitor patient progress and adjust treatment plans as appropriate
- Supervise and collaborate with Physical Therapist Assistants (PTAs) in accordance with regulatory requirements
- Educate patients and caregivers on therapeutic exercises, mobility strategies, fall prevention, and safe home practices
- Coordinate care with physicians and interdisciplinary team members
- Participate in case conferences and care planning discussions
- Complete evaluations, progress notes, and other clinical documentation accurately and within required timeframes
- Support discharge planning and continuity of care
- Participate in quality improvement initiatives and ongoing professional development
