Job Summary

Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and review of medical records to determine care needs.
- Develop and implement individualized care plans aligned with physician orders and patient goals.
- Manage complex cases involving critical care, ICU, or trauma patients, ensuring timely interventions and monitoring.
- Coordinate discharge planning and facilitate smooth transitions between inpatient hospital stays and home-based care settings.
- Perform diagnostic evaluations, medication management, and wound care while adhering to HIPAA regulations for patient privacy.
- Collaborate with interdisciplinary teams including physicians, therapists, case managers, and social workers to optimize patient outcomes.
- Educate patients and families on disease management, medication adherence, infection control (including infectious disease protocols), and self-care techniques.
Requirements
- Valid RN license with current registration in the state of practice.
- Proven experience in critical care environments such as ICU, Level I or Level II trauma centers, or emergency medicine settings.
- Strong background in hospital medicine, medical-surgical units, or outpatient clinics with exposure to post-acute care or home health services.
- Knowledge of physiology, medical terminology, and diagnostic evaluation methods essential for accurate patient assessment.
- Experience with discharge planning, case management, utilization management, and managing complex patient cases.
- Ability to perform comprehensive patient assessments including vital signs and functional status evaluations.
- Familiarity with HIPAA compliance standards and medical records documentation practices.
- Excellent communication skills for effective collaboration with healthcare teams and education of patients/families.
- Flexibility to handle diverse clinical scenarios such as geriatrics, pediatrics (PICU experience preferred), behavioral health issues, or outpatient services.
